All student-athletes who plan to participate in varsity or junior varsity sports at New England College are required to complete the proper medical forms prior to the first practice in order to be eligible.

The informed consent form states that all student-athletes participating in collegiate athletics are aware of the inherent risks of sport.

The medical history form must be completed and all student-athletes must have a qualifying medical evaluation no more than six months prior to their first practice date.

The student-athlete insurance information form must be complete and returned with a copy of the student-athletes’ insurance card.
